*12  Observed the following improper hygienic practices: 1. employee not washing hands after removing gloves, 2. personal beverages next to single service cups, 3. comb and gloves on shelf with dry goods. Please provide a completely separate area away from all food, food items and food equipment for all personal items. 15  Observed the following non-food contact surfaces to be improper: 1. rust on chest freezer in back room that stores bread, 2. rust and broken lid on chest freezer by back door, 3. large dry store room shelves in poor condition - chipped wood.
